The humanistic side of every being is managed by the needs and wants of that individual. Nevertheless, the scale of the consumer sometimes meets with heavy need, that have not the necessary resources to uphold those needs that each individual or family has. There are 8900 homeless locally; made up of disabled veterans and the elderly and single family households. According to current census population this is 13.2 percent less than that of 2012. Thereof, these target populations; a very small percentage are technology active and the other percentages are either overwhelmed by the abyss of resources that are available, & not easily attainable or they simply have given up to the giant automation known as the “System” .


Most individuals either see there total gross income as above the poverty level or beneath it. However, the actuality is that the Dept. of Health and Human Services that publish the poverty level as set by the U.S. Census Bureau. There are actually 4 levels of the (% FPL) Federal Poverty Level; which would qualify most for, government assistance programs. There is very low income, which could be qualifiable for some health care plan, child developmental and housing and then the low income to Moderate Income #bluecollar and then the 1/10th of a percent wealthy.


Most people know that there are resources available but usually figure that these resources are for the destitute and previous target populations mentioned. Although, this ideology seems to be the consensus, it is not altruistically true, and it has been found that the percentage of people that can qualify for these assistance type programs, gradually outweighs the amount of people that actually apply or are aware of their eligibility. In 2008 there was a major push to have more people sign up for Supplemental Nutrition Assistance Program to further create a stimulus within the economy. Food programs were developed mainly for low income households.


Most government funded programs are issued grants through payments to the state. Organizations such as non-profits and private sectors lobby the Federal government through grant writing; to establish a public aid fund. The funds are further broken up to fill the gaps in socio-economic programs, thus established by gubernatorial aid.

As the individual or family in need of housing assistance, utility assistance, food assistance and any other basic need thought of; the fogy swarm of resources become a part of a defunct system too difficult to navigate through with out the guidance of a case care manager or information worker of some sort. Thereby; the need of, non-profit community based organizations & agencies whom serve as a beacon through the fog. Thus by, providing information – advocacy & support; so that actions can be taken to help theses individuals or families become self sufficient.

Case Care Management Part 2

Take Action, Get Involved, Initiate The Interaction & Engage the Flow Of Energy

The biggest barrier for any activist while conveying a block of information to their target audience is to overcome the “I Don’t Know How!” and instigate a reaction so that there is some sort of action taken.

As an inherited responsibility to care for our planet and all the innocence with-in it or to be compassionate for all the human beings that inhabit or that you may come into proximity with; we have to get involved. Even a micro-organism has some sort of action it takes to ensure its productivity in nature, this is what’s known as the phenomenon of spirit. Hence, we must infer that the community gatherings for social unification and the fundraisers for community based programs require group participation and group participation only requires 1 individual to initiate action to solidify the group reaction and community response.

Foster Youth Awareness Month [April]

There are many reasons people choose to get involved and what that reason is really does not make much of any difference as long as an action is being taken, towards the benefit of, that cause. Inaction benefits no one, and if anything it defeats the purpose of spirit within nature. This month was awareness month for foster children. April was awareness month for children in general. April being the month in which children are recognized with their own day.
We must conclude that the purpose of community is the recognition of childhood innocence and achievements. Furthermore that, a childhood needs to be recognized and the responsibility of adulthood is to make further lead way for that child to succeed at every endeavor.

Event Operational Strategy June 2014

Target Audience

Outreach and Education is a key concept of providing information about specific topic. The ability to provide information relies primarily on the subject of discussion that can be communicated to your target audience.
Key points would be to determine what the scenario by using probing questions to find the primary concern or need and then any other sub sequential unstated need. Probing is a key concept while providing outreach, however there are various techniques for using probing questions such as; emphatic listening: where you allow for your target audience to present their concerns while offering minimal encourages to steer the conversation into a possible end result. Question and Answer: when you provide a possible end result through assessment questions. Narrative, would be another type of outreach tool to engage your target audience in discussion by providing all of the information at once with loaded questions, so as to inspire questions from your target audience.
The target audience will determine what materials are needed during the outreach and education event. Knowing the demographic information prior to attending the event will help the stakeholder be better prepared for the diverse groups that the information will be conveyed to. By providing an individual advocacy for the individual whom was unaware of the message, provides your target audience with a sense of empowerment to better the information being offered.

Outreach Techniques

Question and Answer
This technique is the most commonly used in an outreach and Education event. Guiding the information receiver to expand on answers to questions of Who? What? When? Why & Where? Can be empowering to that individual, so that they may reflect on their current needs and then inquire further.

This technique allows the sender of information to touch on key milestones, allowing the receiver to gather the crucial information that they are listening for and then ask questions to further understand some of the information already provided.

Both of these techniques require assessment questions to get a better picture of the target audience’s situation and then appropriately provide answers to either the stated needs or unstated need of the individual, family or group.

Case Care Management Scenario

Case Management can be handled at any level of a hierarchy, and along various different platforms. It does not matter if it be face to face or through some sort of tele-communication device

In this scenario I will establish the syllogism that is needed to resolve the breakdown in communication from an agency to client. Being able to provide updating information to your client about services rendered is a key functionality of any well greased operation. We can further view the breakdown that is mostly apparent in any written communication due to the affluence of semantic and dialect. On a daily basis I have viewed many instances where the client whom has received a letter informing of changes carry the simplest message in the most difficult language. Most agencies provide there information in a format that is suppose to sound professional so that at a common level it may be understood as policy. Nevertheless, the breakdown occurs when the message is lost while the receiver is attempting to decipher the information provided.

1) A client receives a letter stating that there is a need to report any changes that may occur with the client that could cause a change to the services provided.
2) The client then interprets this information to say that; the need to report changes is needed and thus follows up with this communication to report that there have not been any changes.
3) consequently the delivery of null information that a need to report changes causes confusion of service at the client level. Thus, generating a blog of clients that are requesting further explanation of this null information.

In this example, of mis-communication we can see the (WBS)Work Breakdown Structure of correspondence and the need for a pool of information workers that provide guidance to the client whom is frustrated! confused! even distraught! and in fear! of the loss of service.
As a client I can empathize with the information worker that received my misunderstanding of the message, because of the simplicity of information being provided could have been delivered at a more common level of language.
As an Information worker I can empathize with the client that misconstrues the message being provided as a request rather than a delivery of information about services. Merely, because of the empathy for that client and their need of the service the agency is providing.
At the Agency level it would be necessary to understand this confusion and thus deliver the message of changes with a more simplistic language that could be comprehensive to different communities that are diverse in nature. Yet still maintain a non-biased and professional tone.

The resolution to this scenario at the client level is to simply continue to ask questions about services. The solution at the Information worker’s level is to attempt to connect with the client through empathetic statements and empowerment techniques to minimally encourage an understanding of the message being delivered. Consequently, at the Agency level a better communication of service to the client can be provided as first intended
